Frequently Asked Questions:
Still have questions about HSA and LMN? Please take a look at our FAQs or reach out anytime. If you’re feeling ready, contact US for a personalized consult.
-
A: Yes, for our integrative services. We have established a dedicated clinical merchant account and separate booking location explicitly for health savings and flexible spending accounts. To ensure your funds are used legally, your sessions must be treated as a medical necessity to address a specific diagnosed condition.
-
A: An LMN is a formal letter written by your licensed healthcare provider (such as your physician, physician assistant, or nurse practitioner). It states that our therapeutic yoga or kinesiology services are appropriate for a specific medical diagnosis designated by your physician (e.g., chronic back pain, osteoarthritis, post-orthopedic rehabilitation, or post-cardiovascular rehab).
Click the button on the left to download our LMN form. Then request to have your physician complete it before your first session with US.
-
A: Follow these three simple steps:
Schedule your initial clinical evaluation with us to outline your functional goals.
Download our Physician Referral / LMN Template [click the button on the left side of the screen] and bring it to your primary doctor.
Have your doctor fill out the form, including your specific ICD-10 diagnosis code, and submit it back to us or keep it for your tax records.
-
A: We operate as an out-of-network provider. While we do not bill insurance companies directly, our services are backed by a National Provider Identifier (NPI).
Upon request, we can provide you with an itemized Superbill containing the necessary diagnostic and procedure codes. You can submit this directly to your insurance company for potential out-of-network HSA reimbursement.
-
A: No. While I hold a PhD in Kinesiology, an MS in Community Health, and a BS in Sports Medicine, I am an academic movement specialist and advanced therapeutic yoga practitioner not a medical doctor, chiropractor, or physical therapist.
Because I do not diagnose medical conditions, prescribe medication, or provide standard physical therapy interventions, all HSA/FSA clients must have a pre-existing medical diagnosis from a licensed provider.
My therapeutic protocols are designed to complement, not replace, your formal medical care. Think of my practice as the scientific transition phase: you receive the medical diagnosis from your doctor, and I design the precise, evidence-based movement therapy to help you safely manage it.
Let’s Work Together.
At Urban Solace, we hold space for the whole person, not just symptoms or goals. Rooted in over two decades of experience in exercise physiology and backed by research and compassion, our therapeutic movement and integrative wellness offerings blend science and soul. This is a space of grace, where you can show up exactly as you are and feel seen, supported, and gently guided toward restoration.
